#1 Cam Lobe Offset

Has anyone come across a camshaft lobe not being central to the tappet guide?
No 1 exhaust lobe (most rearward), is approx 3mm to the left of central, just enough for the lobe to be touching the guide under certain circumstances, and creating a rattle.
Whether this a faulty camshaft I am not sure?
