#1 FHC rear view mirror attachment

Looking for help on how to attach the triangular rear view mirror base to the car. The parts book calls for 3 machine screws. I have 3 holes in the front headliner trim piece and another 3 holes in the body above the windshield opening but no captive nuts or a plate to bolt to. Thanks.

There's supposed to be a plate with 3 threaded holes. It might have slipped to one side or the other...

Thanks. My plate is missing but easy enough to make one when you know what should be there!
