The rear chassis legs have sleeves in them where the V mounts fix for the IRS. Should every single one of those 8 holes have a sleeve in them?

Almost certainly yes.
Otherwise they would crush when the bolts are tightened.

Okay thanks. Mine have one missing on each side in exactly the same location. I couldn't decide if it was meant to be like that or whether they had somehow fallen out. Looks like a tricky repair but do-able.

They are brazed into the steel channel. At least mine were ('68 OTS).
